Do you flush the downpipes as well?
Yes. Clearing the spouting and leaving a blocked downpipe just moves the problem, so both are done.

Can you wash the outside of the gutter too?
Yes, and it is worth doing at the same time. A clean house with a dirty spouting line still reads as dirty.
